Overview of the Process

1. Email nationaloffice@etasigmagamma.org requesting a call with the Director of Chapter Development to discuss the potential opportunity to create a new chapter.
2. Fill out the new chapter application form and submit this application and supporting documents to nationaloffice@etasigmagamma.org and the Director of Chapter Development.
3. The following materials should be included with the completed application:
   • List of 5 petitioning student members; at least 3 of these students need to have 2 or more semesters before graduation
   • A copy of the academic program’s curriculum showing courses that align with the NCHEC Areas of Responsibility for Health Education Specialists and/or CEPH accreditation requirements
   • Three letters of support from the proposed Chapter Advisor, Department Head, and Dean
4. The National Eta Sigma Gamma Board of Directors will vote on the approval of the new chapter petition/application.
5. Upon approval of the new chapter petition, the institution will pay a $600 chapter installation fee, which covers expenses for chapter installation materials.
6. A leadership training with the Director of Chapter Development, the new chapter’s advisor and executive board members will be scheduled and held via Zoom.
7. The formal chapter installation and first member initiation ceremony will be scheduled, and the Director of Chapter Development or another member of the national board will attend via Zoom to officially install the chapter.
